1. What this is
Open-play volleyball is a physical, contact-possible recreational activity. By registering, the participant acknowledges that injury is a risk of playing sport.
2. Assumption of risk
The participant takes part voluntarily and accepts the ordinary risks of volleyball, including collisions with other players, falls, and contact with the ball, floor, net, and standards.
3. Participants under 18
A player aged 16 or 17 may take part only if a parent or legal guardian registers her, accepts this waiver in their own name, and completes payment. Open to all women, ages 16 and up.
4. No medical service
This program is recreational. Nothing here is medical care, medical advice, or an evaluation, and taking part does not create a patient relationship with Monarch Medicine Urgent Care or any of its clinicians. In an emergency, staff on site will call 911.
5. Photography
